Steph Smith relays the framework from Peter Attia's book Outlive. Attia works with people to get them not merely to the fitness level of people two decades younger, but to the elite top 2% of people two decades younger. Steph notes that most people's first reaction is that this is impossible, since they cannot even reach the top 2% of their own age group. Attia counters with cases of people in their 80s, 90s and past 100, including Robert Marchand, who passed away at 109, was still cycling at 101, broke records at 101, then worked with trainers to break his own record at 103, hitting 27 kilometres an hour on a bike, and continued competing until 105.     Define what you want to still be doing decades from now

    The decathlon is a list of concrete physical events for the end of life, not an abstract wish to be healthy. Steph's illustration is arriving somewhere with a VO2 max of 15 and leaving with it at 25 at age 80, a specific capability change rather than a vague goal.

    Set the benchmark two decades down and at the top 2%

    Attia's standard is not the average person twenty years younger, it is the elite top 2% of that cohort. Steph explicitly notes people find this absurd on first hearing, which is the point: it builds in enough headroom to absorb two decades of decline.

    Watch out Do not soften the benchmark to your own age group; that standard bakes in the decline you are trying to prevent.

    Take objective baselines before training

    Steph's own trigger was a Q Bio full-body scan two years earlier that showed high cholesterol despite being naturally thin, with neither parent having high cholesterol. She now does blood work roughly quarterly, tracks her weight daily and logs nearly everything she eats.

    Pro tip Steph estimates her entire tracking stack adds up to roughly ten minutes a week.

    Add a harsh continuous data source

    Use instruments that contradict you. Steph describes DEXA as very harsh, telling people who assume they are 15% body fat that they are really 22%, and CGMs as equally harsh, spiking on a sugary salad dressing or a steak and potato. Her framing is that you cannot hide from the data.

    Pro tip A CGM sensor lasts around two weeks; most people run one for two to four weeks to get their fasting level, spike sizes and area under the curve.

    Train against the benchmark and keep competing

    Marchand did not merely maintain: at 101 he broke records, then deliberately targeted his own record at 103 and kept competing to 105. The training stays goal-directed rather than maintenance-directed for as long as possible.

    Re-measure and adjust

    Repeat the same tests on a schedule so progress is judged against the earlier numbers rather than against how you feel. Steph's point about being naturally thin is that subjective self-assessment was actively misleading for years.

In the wild

Robert Marchand at 101 to 105

Steph cites Attia's example of Robert Marchand, who died at 109. At 101 he was still cycling and broke records, decided that was not enough, worked with a group of trainers, and at 103 broke his own existing record at 27 kilometres an hour on a bike. He kept competing until 105, with YouTube footage of reporters crowding the finish line because he should not have been able to do what he was doing.

A documented case of someone substantially improving measured athletic performance after 100, which is the existence proof the framework rests on.

Steph's own thin-but-unhealthy correction

Sam observes that Steph was naturally thin, and that thin people often assume they are therefore healthy. A Q Bio full-body scan two years prior returned high cholesterol; she assumed genetics until discovering neither parent had it and her less healthy father's was lower than hers.

She rebuilt around objective data, quarterly blood work, daily weight, food logging, a CGM, and roughly 500 running miles in the year, on top of soccer and spin.

Common mistakes

Dismissing the benchmark as impossible

Steph notes the standard reaction is that you cannot reach the top 2% of your own age group, let alone one two decades younger. Attia's counterexamples include people who did not take care of themselves at all before starting a regimen and then completely reshaped their fitness in their 80s and beyond.

Using appearance as the health signal

The thin-equals-healthy assumption is the exact trap Steph fell into. Body shape carries no information about cholesterol, glucose response or VO2 max, and it flatters precisely the people who most need the data.

Collecting only broad, non-actionable data

Steph's complaint about her existing devices, an Eight Sleep, an Apple Watch, her husband's several Oura rings, is that the readouts are so broad you learn you slept badly without learning what to change. The instruments worth adding are the ones that tie a specific input to a specific measured response.
